Two small fish...
When Hemingway, (old man) talks about the two fish that were inside the bigger fish, it makes me believe that those little fish represent just
the few ideas he has for his work. However some of those fishes make it, and some don't.... meaning some ideas reach the potential of putting them down on paper and some just don't. (Just a thought)

i agree, i also think that the fish could represent steps to the big idea. meaning that the little fish are small ideas or eye openers to expand his horizon on the topic, so that when the big fish(big idea) comes to him, he has a better understanding of the idea, of what hes going to write.
