I Feel Like that little boy can symbolize one of his wives, like hes always by the old man's side encouraging him no matter what. And the fact that he hides the picture shows that he doesnt want the boy to know about his past. And the picture being hidden under a clean shirt can show that his memory is still alive of that wife because the story is detailed to the point it says clean but hidden.
